Project snapshot

Lower East Side Girls Club implements a community health intervention providing over a dozen female head-of-household participants with prediabetes or diabetes free, locally sourced produce, prepared meals, one-on-one dietitian counseling, nutrition and culinary education, and integrative metabolic testing (A1C, lipid panels, continuous glucose monitoring) conducted by Weill Cornell Medicine staff. The program integrates food access, clinical monitoring, and skills-based education to improve glycemic and cardiometabolic outcomes, enhance self-management, and evaluate a scalable community-centered model to reduce health disparities and healthcare utilization in a high-need urban neighborhood.
